Job description: An exciting and unique opportunity has arisen for an experienced mental health nurse to be part of the complex psychosis pilot operating between The Assertive Outreach Team and West and North West CMHT.There are 2 parts to this position: initially a 12-month Assertive Outreach Team pilot role clinically based in a CMHT, then a substantive post as a Mental Health Nurse Case Manager within The Assertive Outreach Team.Main duties of the jobAs part of the pilot, you will provide early help and engagement interventions to service users with assertive engagement needs that do not currently meet the threshold for the Assertive Outreach core offer across the West and North West CMHT/Hub.
